Registering with the surgery
To register with the surgery download and complete the GMS1w (PDF) form.
Please complete all necessary fields and ensure that the form is signed before returning it to the practice.
When you register, it’s helpful to have your NHS number. Your NHS number is a 10 digit number that you find on any letter the NHS has sent you. For example, 485 777 3456.
